This is a gentleman who suffered adjacent level spinal stenosis subsequent to a fusion. Worsening spinal stenosis and neurogenic claudication and leg pain drove him to seek an answer. We were able to place a small interspinous spacer above his fusion to decompress his spinal nerve roots. 40 minutes, outpatient, weekend recovery.

    Good job with the lighting. My Nevro stimulator’s leads shifted 3 months after implantation. The rep, who along with the doctors makes a fortune from working for Nevro told me I was still getting coverage over the area but I was not. before I knew this could happen, the stimulator just stopped working. Nobody told me I needed to turn my phone off then back on again to get the app up and running. So that was surgery number two. We tried to get the stimulator tailored to some tingle settings. It does not work. It is either too strong on one side while two week on the other, or simply send a burning sensation down my legs. We tried several times and I have got several x-rays, meaning exposure to who knows how much radiation. Now the battery has shifted because the subcutaneous fat it was placed in is not fibrous enough. So I’m onto surgery number three. There is no company headquarters phone number to reach out to. I have been Stonewald for weeks, trying to reach a manager and finally after screaming at the medical person in charge, she’s connected me with someone who was clueless and evasive who finally gave me an email address to the legal department. I think Nevro is questionable to say the least.

    Back issues since I was 16.Diagnosed with degenerate disc,spinal stenosis,spondy and bone spurs when I was 20 years old.27 now,several injections and now looking into possible surgical options.They want to fuse L3-L5 but I refuseee.I have had it so bad I couldn’t walk,used canes,wheelchairs etc.The body will heal just give it time. The only reason I lasted so long is due to core/glute strength and exercise.Hammer the core strengthen the legs and lower back.Reverse hypers.Stretch the quads massage the psoas pirifimoris etc.I hope I find something to avoid surgery but the massive compression over the past 15 months has me beat.I have tried absolutely everything and done everything to my power.I educated myself read books watched videos etc.. seeing my mother go through several back surgeries and always told myself I never want to experience that kind of pain.Doing all I can.

    Great content, thank you! I had my first block 'test' 2 weeks ago and got lots of relief for the rest of the day so success #1. (Going back in 2 weeks for test #2.) I have pain that radiates down into my buttock and into the back of my thigh which you said was more likely stenosis but the facet nerve blocks worked so I'm even more confused now, lol. There's also pain in my hip on the side that's not affected by the (sciatica?); nothing shows up on X-rays, CTs or MRIs so I'm wondering if that could be related to me compensating for the back pain when I walk or move. Really wish they'd hurry up and 'discover' comparable treatment for knee pain, although I know mine is due to an assortment of issues in there - torn meniscus, bits and bobs floating around inside the joint, etc. - as I have probably two knee replacements in my future (that doctor appointment is tomorrow).

    Would there be any physical difference in feeling between a SI joint and a Facet joint? I have arthritis of the L5 facet joints, but only have one side having pain, so I’m thinking it might be SI joint (or both’) dunno

    What would be the reason that a regular ablation is still considered above the endoscopic removal is the removal of the nerve is a longer relief up front? Insurance?

    I had the first round cortisone and steroid injection done this way 2 weeks ago , I could feel the difference within 30 minutes. Booked for another injection in 3 days, my neurosurgeon said it should keep me pain free for up to 18months. Fingers crossed. I have also started physiotherapy to strengthen back muscles.

    Omg it was very good explanation. I was told i need L2 to L4 laminectomy with right sided fasectectomy, but my extremely pain is from my SI. I have exactly all the symptoms you mentioned and going to the bathroom is my big nightmare....

    Pelvic instability is so bad that I'm using a wheelchair. It buckles with the smallest movement. It feels like I'm on a boat just sitting. Car accidents (tboned 2x), falls. It buckles and I fall. Usually on my butt. That's why I use a wheelchair. Stabbing pain used to cause my legs to buckle making me fall. I decided to use a wheelchair to avoid the pain and falls. I've done a year of PT, pain shots, imaging. And the doctor who was going to do ifuse just left the hospital. So I'm waiting for a different neurosurgeon to evaluate and schedule. It's like my upper half and lower half work like normal independently - but they don't feel connected in the hips.

    I’m so miserable I have severe osteoarthritis, I had brunch block yesterday and it failed, this has been going on for 30 years, after test after test, 2 tkr and one that failed and my ankle fused, I’ve got oa all over don’t know what to do

    I'm 69 and going to have a bilateral medial branch ablation at L4, L5, S1 on 6/18/24. The two diagnostic blocks gave me 80 to 90 percent relief. I'm looking forward to real back, buttock, and hamstring pain relief. If i have to have facet joints screwed for fusing, I'm hoping it's a few years out.     I've become a victim of Doctors I have these symptoms including the sciatic pain down to my foot so after 2 discectomies and one Foraminotomy a 2 level fusion with no relief and all the doctors that i have seen not a one of them have ever suggested or even looked for SI joint problems . So last week at a year follow up dr appointment I suggested an injection to the SI joint so I'm still looking for answers. This a great in depth video that i wished that I could have seen 5 years ago. Getting the right diagnosis for your pain is so critical !!

    Hello Doctor. I just watched this video, and you gave me some hope. My last MRI showed that I have severe stenosis, and the Doctor told me that I'm not a candidate for non-invasive surgery, only laminectomy surgery. I would like to explore all my options before surgery, and I will be very interested in one of the last treatments. Do you know an orthopedic surgeon performing these treatments in Kansas City?     I HAD MORE THAN 15 OF THE BACK INJECTION WITH MANY RADIOLOGIST DOCTOR ON SCANNER:- I NEVER EVER HAVE HAD ANY PROBLEM OR SIDE EFFECT. AND THE AVERATE TIME OF RELIEF IS AOBUT 4 MONTHS!!! IF THE INJECTION IS DONE BY EXPERIENCED RADIOLOGY DOCTOR AND ON SCANNER; MOSTLY THAN NOT =(( NO PROBLEM AT ALL)!

    I wish you were my doctor, I've had 2 compression fractures and first he didn't believe me, then sure enough there was the fracture. Never said anything about this. ONLY---get a cortisone injection!

    Ive been suffering from this horrible pain for 14 years. All my Mris and X rays come out clean. I just want this godawful pain to go away. It's robbed me of my 20's, a mans prime. My injury happened after lifting something. Life is too short for me to live it this way. Its 3am and cant sleep 😔. My mental strenght to go hold on is chipping away.

      Have you been to an Orthopaedic Doctor? I go in July to ck out my options. I TOTALLY UNDERSTAND about quality of life. I've been living like that for over 15 yrs. Gets old!!! I had to pursue things myself and totally bypass referrals.

      @Merri1961 yeah I went to 4 already. None have figured it out. My chiro saw my mris and they're pretty normal. He mentioned that I have Si Joint dysfunction. I always have weird pops I'm my low back when shifting positions.

    Doctor, I’ve decided to start with physio first and haven’t done RFA yet. It has been 3 months since it is painful to stand, however I’m ok with tolerating this pain (I reduce standing where possible, to have less flare ups and exercises don’t hurt my facets) as I want to see the actual improvement in my pain and standing pain free by working on my muscles and understand that this is a long process. However I started to think if that pain and current inflammation after standing will cause more damage and I should do RFA and then continue with exercises and work outs. Could you please advise on what would be better? I wanted to start with FRA later, when pain is stronger (I’m 35 y.o.).

    Wow wow wow - how educational. Had an MRI yesterday, found out about my facet problem. This is so informative! My ex died years ago after a neck operation so of course I’ve been nervous of course. But I am in much better shape than him so will actively work on PT.

    The problem is these so called interven doctor r walking patients who do not want to have with you hret thll re dose ther pain meds this is a problem iv faced many times so it must b across the brod by intervenil pain doctors who clam pain management n spine when many u won't even see a doctor. This is frude mostly Medicare n they need punishment

    Wow thanks for all this info in 2024. Im in UK have lumbar stenosis & cannot stand for more than 2 mins or walk fir more than 50 yds without severe pain. My GP has prescribed pain killers that turn me into a zombie so i manage as best i can without it. They also have sent me to physio therapist at hospital. Does not seem to be improving anything. As usual sitting or laying improves the situation. I often bend over for some relief if im on my feet & out maybe around supermarket. Im 73 now & had this fir past 5 yrs. Wont be long before im totally in a wheelchair! 😢

    Thanks doctor. I have stenosis in C4-C5, C5-C6, L3-L4, L4-5 and L5-S1. Pain in neck but minor symptoms in my arms. Lumber pain in left buttock. Both shins are tingling and left foot tingles.
